Miss Donna Formby, age 62 of LaFayette, GA passed away Thursday, January 5, 2023 at a Chattanooga area hospital.

Miss Formby was born July 11, 1960 in Cedar Town, GA to the late Cecil and Claudia Bryant Formby.

She is survived by her caregiver, Gina Tucker; along with her friends from her home and the day program.

Graveside and interment services to celebrate the life of Miss Formby will be held at 3:00 PM Wednesday, January 11 at West Hill Cemetery with Rev. Randy Harden officiating. Pallbearers will be Derrick Henton, Dillard Henton, and Gage Taylor.

Visitation will be at Mason Funeral Home Wednesday from 1:30 – 2:30.
